What are your views on the ACCORD and ADVANCE trials?

Jeff, Your assessment of the results of the ACCORD and ADVANCE trials is on point. The key is that in patients who are at high risk, agressive, rapid reduction in A1c may expose patients to increased risk. The point would be as implied by the ADVANCE trial, to start early and keep the A1c as near to normal as possible before serious micro and macrovascular complications have developed. I am reminded of the old saying that seesm to still hold true. "An ounce of prevention is worth more than a pound of cure."
